Service Agreement- Company  This is a Service Agreement which your institution needs to complete and return to AISS. This is a general Release Form that we use for any investigations.
Service Agreement- Individual  This is a Service Agreement form for the individual requesting the services. This general Release Form, that we use for any investigations, needs to be completed and returned to AISS.
Texas Sales Tax Exempt  If you are a Non Texas Individual or Non Texas Business, and the Subject of the Investigation is NOT IN TEXAS, you must fill out the Texas Sales Tax Exemption Form to be exempt from the 7.25% Sales Tax for work performed in Texas. Please fill out and fax to AISS.
Open Billing Agreement- In some instances, AISS will allow open billing with a credit card on file.
Credit Card Authorization Form- This will authorize AISS to process a charge on your account.
F.C.R.A. Compliance Form- This is the form which your institution needs to fill out and return to AISS. This sets the guidelines for complying with the Fair Credit Reporting Act (FCRA), which includes Pre-Employment Investigations.
Adverse Action Letter- This is the form letter that should be given to any applicant who is rejected due to derogatory information found during the background investigation.
Consumer Summary of Rights- This should also be given to each applicant who is rejected due to derogatory information found during the background investigation.
Pre-Employment Background Request Form- Your applicant in question should fill this out when applying for a job. This is the release form that we need to have on file to conduct the Pre-Employment Investigations. The bottom portion should have your company name in the blanks provided.
Pre-Employment Credit Report Release Form- Your applicant in question should fill this out when applying for a job ONLY IF A CREDIT REPORT IS NEEDED. This is the release form that we need to have on file to conduct the Pre-Employment Credit Report. The standard release form above will not give authorization for a credit report. This is the form that we need to have faxed to us. It will open as a Microsoft Word Document. If you have trouble opening it, please call 281-397-9535 and we can fax one to you.
Surveillance Contract- This will outline all terms of a Surveillance Investigation or a case on retainer. This must be completed and on-file before any investigations may begin.
Lab Work- Contract of Services- This will outline all terms of service for Laboratory Tests. This must be completed and on-file along with payment in full before any laboratory tests begin.
The Nature of Surveillance and Investigations- This is a brief introduction on what to expect during an investigation or surveillance.
